Indications of Cef-3

Upper Respiratory Tract Infections (URTI): Prescribed for otitis media and other upper respiratory tract infections (URTIs) when the causative organism is resistant to other commonly used antibiotics or when there is a risk of significant treatment failure.

Lower Respiratory Tract Infections (LRTI): Commonly prescribed for its effectiveness against the bacteria responsible for causing bronchitis.

Urinary Tract Infections: Prescribed for the treatment of urinary tract infections, including cystitis, cystourethritis, and uncomplicated pyelonephritis. It has demonstrated clinical efficacy against commonly occurring pathogens such as Streptococcus pneumoniae, Streptococcus pyogenes, Escherichia coli, Proteus mirabilis, Klebsiella species, Haemophilus influenzae (both beta-lactamase positive and negative strains), Branhamella catarrhalis (both beta-lactamase positive and negative strains), and Enterobacter species. Additionally, Cef-3 remains highly stable in the presence of beta-lactamase enzymes.

Gonococcal Urethritis: Effective against Neisseria gonorrhoeae, the bacteria responsible for causing gonorrhea. By targeting and eliminating the infection, Cef-3 aids in resolving gonococcal urethritis and helps alleviate associated symptoms. 

Acute Otitis Media: Prescribed for the infection of the middle ear that causes pain and discomfort.

Pharmacology of Cef-3

Cef-3 is a third-generation cephalosporin that effectively treats bacterial infections caused by susceptible Gram-negative and Gram-positive organisms. It demonstrates high stability in the presence of beta-lactamase enzymes, making it effective against bacteria resistant to penicillins and certain cephalosporins due to beta-lactamase production. Cef-3 works by inhibiting mucopeptide synthesis in the bacterial cell wall, leading to its antibacterial effect.

Dosage & Administration of Cef-3

Dosage guidelines for different age groups of Cef-3 are as follows:

Cef-3 Powder of Suspension:

For children, the recommended dosage of Cef-3 is 8 mg/kg/day, given either as a single dose or divided into two doses. However dosage for children varies based on age:

  • Children of age less than 6 months: The safety and effectiveness of cefixime have not been established in children below 6 months of age.
  • Children of age 6 month-1 year: 3.75 ml or 75 mg.
  • Children of age 1-4 years:  5 ml or 100 mg.
  • Children of age 5-10 years: 10 ml or 200 mg.
  • Children of age 11-12 years: 15 ml or 300 mg.

Cef-3 Capsule/Tablet:

  • Adults and Children over 12 Years: 200 mg – 400 mg, as a single dose or in 2 divided doses daily for 7-14 days, according to the severity of infection.
  • Elderly: The dosage for elderly patients is the same as that for adults, and no special precautions are required. There is no need for dose adjustment based on age alone.

Direction for Reconstitution of Cef-3 Suspension

  • Cef-3 Powder for Suspension:
    • To prepare 30 ml suspension, 20 ml boiled and cooled water is required.
    • To prepare 40 ml suspension, 25 ml boiled and cooled water is required.
    • To prepare 50 ml suspension, 35 ml boiled and cooled water is required.
    • To prepare 75 ml suspension, 60 ml boiled and cooled water is required.
  • Cef-3 Paediatric Drops:
    • To prepare 21 ml suspension, 14 ml boiled and cooled water is required.
  • Cef-3 Forte Powder for Suspension:
    • To prepare 50 ml suspension, 35 ml boiled and cooled water is required.

Information for the Patient

  1. Shake well before using the suspension.
  2. Don’t discontinue the therapy suddenly without consulting your doctor.
  3. Discard the unused portion of reconstitution suspension after the time mentioned above.
  4. Keep away from the reach of the children.

Adverse Reactions of Cef-3

Cef-3 is generally well tolerated, with the majority of adverse reactions reported in clinical trials being mild and self-limiting. Gastrointestinal disturbances such as diarrhea (severe cases may require discontinuation of Cef-3), changes in stool color, nausea, abdominal pain, dyspepsia, vomiting, and flatulence have been observed. Central nervous system disturbances like headache and dizziness may occur. Hypersensitivity reactions are possible but typically resolve after discontinuing the therapy. Infrequent and reversible hematological changes and elevation of serum amylase have also been reported.

Precautions & Warnings of Cef-3

Caution should be exercised when prescribing Cef-3 to individuals with a history of gastrointestinal diseases, especially colitis. Dosage adjustment is only required in cases of severe renal failure, with a creatinine clearance below 20 ml/min.

Pharmaceutical Precautions of Cef-3

Capsule and Tablet: Store below 30°C and away from direct sunlight and moisture.

Powder for Suspension/ Paediatric Drops: Store below 25°C, protected from light and moisture.

After reconstitution, the suspension/drops can be used within 7 days if kept at room temperature and within 14 days if kept in the refrigerator (2°-8° C). Always keep the bottle tightly closed.
